AS per the OMHA Vaccination Policy, we are required to ask for proof of full vaccination in order for your player’s registration to remain valid. We have been asked by OMHA to NOT collect personal documentation but they do require us to see that documentation so we can record it on a spreadsheet and keep your player’s registration valid.
This is applicable for all players born in 2009 or earlier, Coaches, Trainers, Managers and Volunteers within the Almaguin Minor Hockey Association.
IF and ONLY IF you feel comfortable to VOLUNTEER your vaccine receipt/documentation proving your player has had the Full Dose of the Covid Vaccine, you can email this to Cathy McNichol at [email protected]. Once she has recorded that she has seen it, your documentation will be deleted.
If emailing your documentation is not an option for you, please contact Cathy via email or talk to your team manager and we will set up a time so you can show your documentation in person. We are reminding you that we are not writing down or keeping any personal information written on the vaccination receipt. We are simply putting a checkmark beside your players name to validate their registration.
